Registration


There is a $12.00 registration fee for the Southwestern New Mexico Regional Science and Engineering Fair.

The following forms are required by all exhibitors:

2009 Registration Form: for the Southwestern New Mexico Regional Science and Engineering Fair.

Checklist for Adult Sponsor:  (Form 1) This form is required for ALL projects and must be completed before experimentation.

Student Checklist and Research Plan:  (Form 1A) This form is required for ALL projects.  A complete Research Plan must accompany Checklist for Student.

Approval Form:  (Form 1B) This form is required for each student, including all team members.

Regulated Research Institutional/Industrial Setting Form:  (Form 1C) This form must be completed after experimentation by the adult supervising the student research conducted in a regulated research institution, industrial setting, or any work site other than home, school, or field.

 Exhibitor Liability and Publicity Release Form:  Please return this with all Southwestern Regional Science and Engineering Fair Registration Packets.

 

The following forms MAY BE required for your Science Fair Project.  Use this worksheet to determine which additional forms you may need to submit. You may need to contact your IRB for more information.

Qualified Scientist Form:  (Form 2) This form may be required for research involving human subjects, vertebrate animals, potentially hazardous biological agents, and DEA-controlled substances.  To determine is applicable, ask the IRB.  This form must be completed and signed before the start of student experimentation.

Risk Assessment Form:  (Form 3) This form is required for projects using hazardous chemicals, activities, or devices.  This form must be completed before experimentation.

Human Subjects: (Form 4)   This form is required for experiments that involve human subjects.  If a surevy of questionaire is used, it a copy of it must be attached.

Vertebrate Animals:  (Form 5)  This form is required for all experiments involving vertebrate animals (humans excluded).

Potentially Hazardous Agents:  (Form 6A) This form may be requried for any projects that include microorganisms, rDNA, or fresh tissue, blood, or other body fluids.

Human / Vertebrate  Animal Tissue: (Form 6B)  This form is requried for any projects that include microorganisms, rDNA, or fresh tissue, blood, or other body fluids.

Continuation Project: (Form 7)  This form is required for any project which is carried on from a previous year.
