Student Guidelines

There is no registration fee for the Southwestern New Mexico Regional Science and Engineering Fair this year.

The following forms ARE REQUIRED by all exhibitors:

SWNMRSEF Registration Form 2017 for the Southwestern New Mexico Regional Science and Engineering Fair.

Liability Publicity Release, PDF  This form is required for each student, including all team members.

Checklist for Adult Sponsor 1, PDF (Form 1) (Form 1) This form is required for ALL projects and must be completed before experimentation.

Student Checklist & Student Research Plan Guidelines, PDF (Form 1A) This form is required for ALL projects.  A complete Research Plan must accompany Checklist for Student.

Approval Form, PDF (Form 1B) This form is required for each student, including all team members.

 

PLEASE NOTE:  All students must also submit a RESEARCH PLAN and an ABSTRACT.

The RESEARCH PLAN should be written before experimentation, and include:

A.  Question or Problem being addressed

B. Hypothesis / Goals

C. Description in detail of the procedures, experimental design, and data analysis

D. Bibliography (List at least 5 major references (e.g. science journal articles, books, internet sites) from a literature review.  If you use vertebrate animals, one of these references must be an animal care reference.

The ABSTRACT should be written after experimentation, should be a maximum of 250 words, and should include the purpose of the experiment, the procedure, a description of the data collected, and conclusions made about the experiment. The Official Abstract and Certification Form can be found here.

The following forms MAY BE REQUIRED for your Science Fair Project.  Use  this link to determine which additional forms you may need to submit. You may need to contact your IRB for more information.

Regulated Research Institutional/Industrial Setting Form, PDF (Form 1C) This form must be completed after experimentation by the adult supervising the student research conducted in a regulated research institution, industrial setting, or any work site other than home, school, or field.

Qualified Scientist Form, PDF (Form 2) This form may be required for research involving human subjects, vertebrate animals, potentially hazardous biological agents, and DEA-controlled substances.  To determine is applicable, ask the IRB.  This form must be completed and signed before the start of student experimentation.

Risk Assessment Form, PDF (Form 3) This form is required for projects using hazardous chemicals, activities, or devices.  This form must be completed before experimentation.

Human Subjects, PDF (Form 4)   This form is required for experiments that involve human subjects.  If a survey or questionnaire is used, a copy of it must be attached.

4 Human Consent Extended Form, PDF This form may be required for experiments that involve human subjects. There should be one form per each human participant.

Vertebrate Animals, PDF (Form 5A &B)  One of these forms are required for all experiments involving vertebrate animals (humans excluded).

Potentially Hazardous Agents, PDF (Form 6A) This form may be required for any projects that include microorganisms, rDNA, or fresh tissue, blood, or other body fluids.

Human/Vertebrate Animal Tissue, PDF (Form 6B) This form is required for any projects that include microorganisms, rDNA, or fresh tissue, blood, or other body fluids.

Continuation Project, PDF (Form 7)  This form is required for any project which is carried on from a previous year.
